The 9 Year Follow-up survey of Prospective Urban Rural Epidemiologic Study (PURE, 2018)

Narrative Description of Project: The main purpose of the study is to find out the differences in risk factors and disease in urban and rural populations and will be conducted in 15 countries. In addition to that the study will focus to understand the state and determinants about health related issues in Rural and Urban areas in Bangladesh and also to examine fatal and non-fatal events experienced by the participants from the time of first follow-up phase in 2008 and its another objective is to examine the utilization of health services by the participants.

 

This will be the third round of follow-up data collection and computerization services under the Prospective Urban Rural Epidemiologic Study (PURE). The goals and objectives of the study are similar to the previous follow-up studies. The 9 year follow-up aims to examine fatal and non-fatal events experienced by participants since the last follow up in 2015 and also to examine the utilization of health services by the participants. The follow-up studies will help in achieving the key goal of the PURE study which is to delineate the key determinants of chronic diseases and how these are influenced by the changing societal factors across the globe. 

 

The Bangladesh Risk of Acute Vascular Events (BRAVE) study has projected the Bangladeshi population as being one of the populations most at risk of developing CVD while also being one of the least studied South Asian countries in this regard. Therefore the PURE project will help to examine the key determinants of CVD in a growing economy like Bangladesh and also how the determinants might vary across a rural and urban population in the country.

 

Description of actual services to be provided by Mitra:

 

The data will be collected by administering questionnaires and will comprise of the:

 

  • 1. Household Contact Form
  • 2. Participant Contact Form
  • 3. Event Reports
  • 4. Verbal Autopsy
  • 5. Health Services Research

 

The assessment is likely to take 45-60 minutes per participant. Data will be collected through questionnaire administration, anthopometry and spirometry measurements. At first, the consent clause will be slowly read to them and only when the participant agrees to the content of the clause and signs it, the interview will be conducted. All data will be collected by trained staff using standardized protocols.

 

Study population: Since this a follow-up study, only the participants who were assessed during the baseline survey will be involved in the study population for the follow-up phase. Therefore in the follow-up we will assess the enumerated 2934 participants of the baseline survey only. 

 

Pre-testing and finalization of Questionnaires: A pilot study will be conducted among 100-200 follow-up respondents to explore the feasibility and acceptability of the interview, the informed consent clause and to test the study logistics.

 

Instruments: Spirometer: It is a device for measuring the air capacity of lungs through measurements of inspired air and expired air volumes. In addition to administering questionnaires, spirometry measurements will be taken for each individual to determine lung function. Validation will be run for all 85 communities before using the instruments in the field. People will be trained to use the device and each of them must do a self-assessment first to pass the competency test. The reports from these tests will be checked by a respirologist and these will be done before the trained individuals are sent out into the field.

 

Data processing and analysis: The data will be analyzed through statistical programs to assess the relative contribution of lifestyle changes over time in individuals and, community level changes in societal domains on the risk factors and their individual and dual effects.
